Home Health Aide - Maplewood, MN
- We are seeking a compassionate Home Health Aide to provide essential care and support to individuals in their homes. The ideal candidate will assist with daily activities, personal care, and ensure the well-being of our clients.
- Responsibilities
- – Provide assistance with personal care tasks such as bathing, grooming, and dressing
- – Assist with mobility and physical therapy exercises
- – Prepare and serve nutritious meals according to dietary needs
- – Perform light housekeeping tasks to maintain a clean and safe environment
- – Monitor and report changes in the client’s health status to the appropriate healthcare professionals
- – Offer companionship and emotional support to clients and their families
- Qualifications
- – Experience in dementia care, home care, senior care, or related fields is preferred
- – Knowledge of medical terminology and basic healthcare practices
- – Ability to follow medical documentation procedures accurately

Job Coach - Coon Rapids, MN and Minneapolis, MN
- Lotus Care LLC is recruiting a Male Employment specialist.
- Employment Supports are provided to vulnerable adults who successfully live on their own with varying levels of support. An employment support Specialist worker provides this support by working 1-on-1 with the person through a series of weekly meetings.
- The employment specialist should be holding an industry-recognized credential from any of the following:
- The Association of Community Rehabilitation Educators (ACRE)
- The National Certified Employment Support Professional (CESP)
- The Direct Support Professional Specialist in Employment Support credential (DSP-S-ES)
- Starting Pay: $24.00 – $26.00 per hour (based on experience)
- Hours are typically Monday through Friday or weekend based on scheduling with the staff and individual.
- Locations:
- – Male client living in Maplewood
- – Male client living on Coon Rapids
- – Other locations as needed.
- Autism or Developmental disability skills experiences preferred. We are willing to train each employee upon hire.

Behavioral Specialist - Nowthen, MN and Blaine, MN
- Lotus Care LLC is recruiting a Behavioral Support Specialist to join our team of independent living skills / In-Home Support services
- Location: – Nowthen and Blaine
- Starting Pay: $22 – per hour (based on experience)
- Nowthen Shifts : M-F 03:30 pm 08:00 pm; Saturday-Sunday 08:00 am TO 05:00 PM
- Blaine: Flexible.
- Under supervision, IHS W Training provide services to adults with Developmental Disabilities, Mental Illness, and/or Chemical Dependency in the individuals home, ensuring that programs are implemented properly to increase the persons served independence. The incumbent will carry out duties and responsibilities in accordance with the organization’s policies and procedures, applicable to federal, state, local standards, guidelines, and regulations as directed by the reporting supervisor.
- Behavioral Essential duties
- Provide direct care and support to individuals with autism and related developmental disabilities.
- Implement behavior intervention plans under the supervision of a Board-Certified Behavior Analyst.
- Assist with the development and implementation of individualized treatment plans.
- Conduct data collection and analysis to track progress and outcomes.
- Assist with crisis management and behavior support as needed.
- Collaborate with a multidisciplinary team to ensure coordinated care.
- Provide support during community outings and social activities.
- Requirements
- Previous experience working with individuals with autism or related developmental disabilities preferred.
- Knowledge of behavioral health principles and techniques
- Ability to follow behavior intervention plans and implement strategies effectively.
- Strong communication skills, both verbal and written
- Ability to collect data accurately and maintain detailed records.
- Basic understanding of medical terminology and ability to take vital signs if required.
- Familiarity with special education practices and principles is a plus.

LPN
- Position: LPN
- Schedule: 40 Hours per week 7:00 AM to 03:00 PM
- – 40 Hours Admin schedule
- Supervisor Duties:
- -On call for AM shifts.
- -Responsible for finding coverage for AM shifts that someone calls off.
- -If you cannot find someone to cover, you will have to cover the shift yourself.
- -Update schedules on Therap and sling as needed AM shift only,
- -Communicating with the training coordinators on training and schedule changes.
- -Attending weekly meetings with the teams (1-3 a week). (1-6 hours weekly)
- -Attend staff meetings all 3 once a month (4-6 hours a month)
- -Review annual and semiannual paperwork (Every 6 months for each client)
- -Conduct staff integrity checks
- -Review tlogs and GERs (Daily)
- -Weekly meeting with Owner and HR
- -Weekly meetings with training coordinators (3 hours weekly)
- – Initiate BIRF
- -Call staff to pick up the shifts. Use the on-call list, ask if staff will cover 4 hours of the shift and someone else the other 3 or 4 hours.
- *Splitting shifts will make it a little easier to cover and if you have to cover, you will not have to
- Skilled Duties:
- – Medication management
- – Provide skilled nursing care to patients in their homes (Bowel program, irrigation program and catherization)
- – Conduct initial assessments and develop individualized care plans
- – Administer medications and treatments as prescribed by physicians
- – Monitor and evaluate patient progress and adjust care plans as needed
- – Educate patients and their families on disease management and self-care techniques
- – Collaborate with other healthcare professionals to coordinate patient care
- – Document patient information, treatments, and outcomes accurately and timely
- Requirements:
- – Valid Registered Nurse (LPN) license in the state of Minnesota
- – Minimum of 2 years of nursing experience, preferably in a home health setting
- – Proficient knowledge of physiology, medical terminology, and disease processes
- – Ability to lift and transfer patients safely
- – Familiarity with ICD-10 coding and documentation requirements
- – Experience with procedures such as gastric lavage and airway management
- – Knowledge of laboratory tests and interpretation of results
- – Strong interpersonal skills and ability to provide compassionate care
- – Health coaching skills to support patients in managing chronic conditions
- – Knowledge of primary care principles and practices
